VARIOUS CONSULATE FORMS

We provide consulate services in addition to processing tourist and business visas. Please refer to our fees listing for the costs related to the following forms. Each of the following forms are in Microsoft Word format (DOC).

⇓ Visa Pick-Up Authorization (.doc)
Use this form if the consulate has mailed you a letter stating that they have your documents have asked you to utilize a visa agency to pick up your application. You can fax us this form or email it to visas@brazilfiesta.net

⇓ Laissez-Passer Form (.doc)
This form must be submitted with visa applications for citizens of Bhutan, Central African Republic, Comoros, and Taiwan. Visas are issued on a laissez-passer basis to citizens of these countries.

⇓ Child Authorization Form (.doc)
This form should accompany visa applications for minors traveling alone or with only one of their parents. The letter should be notarized prior to submission.

This form is for individuals applying for a wedding certificate within Brazil.

Renovacao de Passaporte Brasileiro: Cidadaos brasileiros que precisam renovar passporte devem preencher esse formulario, enviando juntamente com o seu passaporte expirado e 2 fotos de passaporte (2x2”).
