The Brazilian Consulate requires that all applications be filled out online at the following URL — these are the only applications accepted for visas. Fill out the online application and print the forms to include with your application request.

VARIOUS CONSULATE FORMS

⇓ Child Authorization Form (.doc)
This form should accompany visa applications for minors traveling alone or with only one of their parents. The letter should be notarized prior to submission.

⇓ Marriage Certificate Form (.doc)
This form is for individuals applying for a wedding certificate within Brazil.

⇓ Brazilian Passport Renewal Form (.doc)
Renovacao de Passaporte Brasileiro: Cidadaos brasileiros que precisam renovar passporte devem preencher esse formulario, enviando juntamente com o seu passaporte expirado e 2 fotos de passaporte (2x2”).
